Mishap Prevention and Organizational Performance
The NSC Mishap Prevention and Organizational Performance (MPOP) Office facilitates organizational excellence across the NASA enterprise and prepares for and helps prevent agency mishaps.
The NSC enables and coaches NASA organizations’ drive toward cultural excellence by
- Enhancing the agency’s ability to identify and mitigate latent and active hazards
- Preparing the agency for mishap contingencies
- Enabling and coaching agency organizations’ drive toward cultural excellence
- Creating awareness of human and organizational dynamics, including Safety Culture and Human Factors
- Assessing organizational risk and safety via Organizational Safety Assessments (OSAs)
- Illuminating the path with lessons learned, including the Apollo, Challenger, Columbia Lessons Learned Program (ACCLLP)
Risk-Driven Planning
While post-mishap work is important, it is only part of what the NSC does. The NSC uses data collected from past mishaps and shares it with the goal of increasing awareness and preventing future mishaps. The NSC is proactive in approaching mishaps and can assist NASA centers with planning drill scenarios and developing Mishap Preparedness and Contingency Plans.
Training and Tools
The NSC supports mishap investigations by providing training, tools and guidance and offers helpful tools related to mishaps, like the Root Cause Analysis Tool, the NASA Mishap Information System, investigation templates and checklists.
Hazard Reporting
The NSC manages the NASA Safety Reporting System (NSRS) : an anonymous, voluntary and responsive reporting channel to notify NASA’s upper management of a person’s concerns.
