Student Volunteer Program Forms and Instructions

Once a student is paired with a mentor it is the responsibility of the student to collect signed copies of the following forms and information. The student should also immediately inform the SVP Manager of who their mentor will be and include the mentor on the email.

All completed forms are to be sent to the SVP Manager in a single email, at minimum, one week prior to the effective start date (address at bottom of page). The program does accept and encourages electronic signatures from all parties if possible. 

Please ensure all of the below are completed and included in your final email submission as instructed. Be advised, PDF forms may not load in your browser, but should be visible once downloaded. If problems persist please contact the SVP Manager for assistance.

  • The Student Volunteer Service Program Agreement (Form LF 457) should be filled out and signed by the student and guardians if applicable.

  • The Student Volunteer Service Working Agreement (Form LF 377) should be signed by the student’s current School Counselor, Academic Advisor, Program Dean or like role.

  • The student must procure a transcript (unofficial is acceptable, as is an acceptance memo if transitioning from high school to college)
  • The student should attach their original cover letter and resume used to find their mentor.
  • The student will also provide their personal information (outlined below) typed into the body of the email, encrypted if possible. If not encrypted, please fill out all the information except the SSN and send via email and call in with the SSN and corresponding name.

On reporting day, the mentor and student should arrange a time at the Badge and Pass Office to escort the student to the work site.

At the end of the student’s volunteer service, the mentor should email the SVP Manager and student a statement describing the work the student performed and the total number of volunteer hours performed.
